Sažetak
Sexual problems are prevalent problems and the prevalence in the general population is: 25% in male and 33% in female. There are several research showing that the same prevalence applies for Croatia. Unfortunately, until 2004, there were no systematic education in sexual therapy in Croatia, and professionals working with patients with these problems had to rely on literature and their own clinical insights. In 2004, the first systematic education in sexual therapy started and 29 people enrolled. In 2007 the Croatian Association for Sexual Therapy was founded. In 2006 two web sites with Q&A possibilities were formed and more then 200 people asked for help. The most prevalent problems in men were: erectile dysfunction (35%), premature ejaculation (23%) and delayed ejaculation (13%). In women, the most prevalent were: anorgasmia (41%) and hypoactive sexual desire (25%). People of all age groups contacted for help. Many of the patients were referred to a sexual therapist and the most prevalent mode of therapy is behavioral approach, with partner therapy. Erectile dysfunction is a problem that often has biological (organic) aethiology (e.g. diabetes mellitus, hypertension, hyperlipidemia). Differential diagnosis relies very much on anamnesis (morning erections, spontaneous erections and erections in masturbation). Treatment can be psychological (sensate focus exercises) and pharmacological (sildanefil, vardenafil). Premature ejaculation is a prevalent problem in young men. It can be treated with behavioural techniques (start - stop) or pharmacotherapy (SSRIs).
