Sažetak
Implanting in alveolar bone areas with unfavorable bone volume represent a special clinical challenge in dental implantology. The limitations of drilling into an atrophic ridge in the maxilla constitute a challenging clinical situation. The frequent problem is bone resorption which reduces the width and the height of the alveolar ridge. If the horizontal bone volume is reduced the implant site can be expanded by bone expansion and bone condensation, making additional augmentation procedures unnecessary. Bone expanders with D-shaped cross-section and of increasing diameters push the bone laterally by separating the labial and palatal bone lamellae. The dentist with surgical experience can achieve excellent predictable results with these techniques, but bone expansion is not suitable for increasing the vertical bone volume. After bone expansion we use bone condensers which due to rounded profiles form the bone cavity for desired implant diameter. Implants can be placed in soft bone without requiring the use of tap. Low resistance during pilot drilling suggests poorly structured bone and it should be condensed with a bone condenser which expands the bone to the corresponding diameter of planned implant. The bone condensers can also be used for an internal sinus lift. The healing phase after bone expansion and bone condensation should be extended to six months.
