Sažetak
Pruritus is the predominant symptom of inflammatory skin disease and can best be defined as a sensation which leads to a desire to scratch. Many chemical mediators of itch have been described or are hypothesized such as histamine, prostaglandin E2, serotonin, opoid peptides, such hachykinins and interleukin-21. The pathophysiology of pruritus is reviewed, showing that itch and pain share the common neuroanatomic pathways but are distinct sensory modalities.
Pruritus, which is the most common dermatologic complaint (more than 50% of dermatologic patients), has an extensive differential diagnosis. It can present with or without skin eruptions. Positive diagnosis is frequently based on careful patient history, physical examination, and labortory evaluation.Primary dermatologic conditions associated with pruritus include inflammatory conditions, infestations or infections, and neoplastic conditions. Systemic disorders associated with pruritus include endocrinologic, hepatobiliary, hematologic and neoplastic disorders, and chronic renal failure. The best results in the management of pruritus are achieved by alleviating the underlying cause. When it is not possible, many other symptomatic treatments are available, that are only partially effective.
