Sažetak
Carbohydrate deficient transferrin (CDT) is one of transferrin isoforms, whose serum concentration increases with alcohol intake of >50-80 g/day and normalizes within 14-17 days of abstinence. The aim of this study was to investigate the reliability of CDT as a marker of chronic alcohol abuse, and to assess its value in treatment monitoring. The concentration of CDT and activities of gGT, AST, and ALT were measured before and on day 7, 14 and 21 after the beginning of abstinence in 20 patients (17 males and 3 females) ingesting >60 g alcohol per day and 40 healthy volunteers (14 males and 24 females). Serum concentration of CDT was determined by CDTect EIA (Pharmacia, Uppsala, Sweden). Activities of AST, ALT and gGT were measured by IFCC proposed methods on a CX4 (Beckman, USA). Results showed that before treatment, the level of CDT exceeded the cut-off value (20 U/L) in 85% of patients. The percentage of pathologic values declined to 65%, 40% and 15% on day 7, 14, and 21 of treatment, respectively. Increased gGT values were found in 60% before and in 55%, 45% and 35% patients on day 7, 14 and 21 of treatment, respectively. The percentage of pathologic AST values 35% declined to 5% of the end of therapy. ALT levels were below the cut-of value throughout the monitoring period. ROC analysis revealed an AST specificity of 100% and sensitivity of 40%, ALT specificity of 98% and sensitivity of 15%, gGT specificity of 93% and sensitivity of 60%, and CDT specificity of 85% and sensitivity of 98%. The results showed the CDT measurement to be the best parameter for monitoring physiologic changes occurring during alcohol consumption and abstinence, and could be recommended in its diagnosis and monitoring.
