Sažetak
Introduction: The CagA producing strains of H. pylori (Hp) are considered to be associated with a more severe gastroduodenal pathology including atrophic gastritis and gastric cancer. Objectives: This study examines the prevalence of Hp infection and CagA seropositivity in group of patients with and without gastric cancer (GC) to determine whether infection with Hp/CagA(+) strains is linked to development of GC. Patients and methods: 25 patients with GC (14M:11F, mean age = 65 yr., range = 45-90), 22 patients with lymphoepithelium lesions (LEL) (11M:11F, mean age = 54 yr., range = 40-67), 49 patients with gastric ulcer (GU) (24M:25F, mean age 62 yr., range = 44-80), 27 patients with non-ulcer dyspepsia (NUD) (13M:14F, mean age 56 yr., range = 45-69), and 33 patients with duodenal ulcer (DU) (15M:18F, mean age = 47 yr., range = 24-73). Hp status was determined with histology, urease, serology (CF, ELISA), and CagA seropositivity was assessed by the presence of CagA IgG antibodies using ELISA (Eurospital, Trieste, Italy). Results: Table shows the prevalence of CagA antibodies in the five groups of patients: Group Age of patients (yr.) Total <50 =/>50 1. GC 1/2 19/23 20/25 (80, 0%) 2. LEL 9/10 10/12 19/22 (86, 3%) 3. GU 5/6 37/43 42/49 (85, 7%) 4. NUD 11/12 11/15 22/27 (81, 4%) 5. DU 19/19 12/14 31/33 (93, 9%) There was no significant difference in the prevalence of CagA antibodies among the five groups. Conclusions: Our results showed that the Hp infection is a risk for GC, but the presence of CagA strain is not the critical factor for the development of GC.
