Sažetak
Aims: To compare the sensitivity (SE), specificity (SP), positive (PPV) and negative (NPV) predictive value of the most widely available invasive and noninvasive tests for diagnosis of H. pylori. Material and methods: CLO test (1 antral biopsy specimen), histology (2 corpus, 2 antral), culture (1 corpus, 1 antral) and serology (CF and ELISA) were used to assess H. pylori status in 1128 untreated patients underwent endoscopy (mean age 51, 6 ą 14, 8 ; 698 male and 430 female ; 897 peptic ulcers and 231 nonulcer dyspepsia). The gold standard was defined by the positive results of any two of biopsy based methods. Results: H. pylori was detected by gold standard in 906 (80, 3%) patients. Conclusion: The results suggest that histology was the most sensitive test for detecting H. pylori, with good positive and the best negative predictive value. Both serologic methods had the lowest combination of specificity and predictive negative value due to a great number of false-positives, suggesting posibility of H. pylori natural clearance in some patients.
