Sažetak
Introduction: Atherosclerotic disease is thought to be chronic and slowly progressive degenerative disease. Atherosclerotic plaques (or atheromas), usually require 10–15 years for full development. We present a case of fast atherosclerotic plaque progression in a 48 years old male patient. Case report: Patient was current smoker with BMI 36 and had all criteria for metabolic syndrome (elevated blood pressure, dyslipidemia, glucose intolerance and abdominal obesity). He was initially treated in our Cardiology department due to acute NSTEMI in Jan 2006. At initial coronary angiography we found 80% proximal ACX stenosis and performed PCI of ACX with BMS implantation. There were no stenoses on other arteries. During the first year of follow up he had good compliance to optimal medical treatment. However, he quit using all medications and was lost to follow-up in 2008 and 2009. He was re-admitted to our department in November 2009 due to troponin-I unstable angina pectoris with no basal ECG changes. We did re- angiography and found normal angiogram of LAD and ACX and subtotal »de-novo« stenosis in proximal RCA that was completely normal 3, 5 years ago. He received PCI with BMS implantation in RCA with optimal result, and had no MACE with good compliance to medical therapy in the last 3 years. Conclusion: Metabolic syndrome probably changed atherosclerosis from slowly progressive disease to highly progressive disease in this patients. There are probably other vasoactive and hormonal promoters of atherosclerosis involved in metabolic syndrome patients.
