Sažetak
Aim: The purpose of the study is to determine the score of leukocyte alkaline phosphatase activity (LAPA) in patients with Hodgkin lymphoma (HL). Hodgkin lymphoma accounts for about 30% of all lymphomas. It is comprised of two disease entities: nodular lymphocyte predominant HL and classical HL. Alkaline phosphatase is an enzyme found in the tissues, serum and mature neutrophils. Low values were found in chronic myeloid leukemia and paroxysmal nocturnal hemoglobinuria while high values can be found in association with inflammation, malignancy and in polycythaemia vera. Methods: Fresh blood smears were stained using cytochemical reagent kit ‘Leucognost ALPA’. The product of positive reaction is brown granular cytoplasmic staining. The score of LAPA was determined in 68 patients (43 female, 25 male). In order to score LAPA, for each smear 100 consecutive band neutrophils are counted and rated by staining intensity from 0 up to 5. Normal score range is 10 up to 100. Results: In all four patients examined at the time of diagnosis, before treatment, LAPA was increased. Among 12 patients examined during chemotherapy, nine had high, one had normal and two patients had low LAPA. Of 47 patients in remission, 12 had high, 33 had normal and one patient had low LAPA. All five patients with relapsed disease had high LAPA. Conclusion: The usefulness of LAPA determination in malignant lymphomas is probably restricted to HL where high LAPA is usually a sign of disease activity and may diminish with remission.
