Human papillomaviruses (HPVs) are very diverse DNA viruses that are strictly species specific. They infect epithelial cells either of the skin or the anogenital and oropharyngeal mucosa. Until now, 130 HPV types have been identified and fully sequenced. Approximately 40 HPV types infect the anogenital tract, of which 15 to 18 oncogenic types are commonly found in anogenital cancer biopsy specimens, notably cervical cancer. The E6 and E7 HPV oncoproteins disrupt and degrade the p53 and pRb tumor suppressor proteins. This allows the virus to force the host cell into cell cycle and thus provide the host DNA replication machinery for viral replication. The pRb inactivation leads to accumulation of p16 that otherwise act as a negative feedback regulator of the cell cycle progression. HPV DNA molecular testing has low positive predictive value for cervical cancer development but high negative predictive value.Alternative biomarkers with higher predictive value to better identify women at greatest risk for developing cancer are being investigated. In this study the immunohistological staining for pRb and p16 was compared on 50 samples of cervical intraepithelial (CIN) lesions (grades 1-3) and correlated with HPV DNA detection. Approximately half of the samples did not have any discernible pRb staining and from those that did 74% had only staining in the basal layer, while 87% had weak pRb staining. Conversely, 62% of samples had positive p16 staining with the majority having intense staining. However, only p16 staining correlated well with lesion grade (Spearman's correlation = 0.729, P<0.0001).
