Sažetak
The study aimed to evaluate the frequency, type and severity of pharmacy interventions and the associated errors that were prevented. In this research, 30 different intervention types were recorded, where more than half of them were rated as major (56.1%). On average, each pharmacist made ~6 interventions (405 interventions per 63 pharmacists), which lasted for less than 5 minutes for more than half of all the interventions (56.1%). When the interventions were judged by apparent harm that would have resulted without the pharmacist intervention, a quarter were judged to be of major harm, whereas 45.3% and 30.7% were judged to be of moderate and minor harm each. In only 8.9% of cases pharmacists thought that no harm would have happened if intervention not made. Majority of interventions were made to inpatients (92.8%) what was expected since pharmacists spent most of their time on wards where most of the interventions were made (89.9%). Therefore, the most frequent sufferers were inpatients who found themselves on surgery and anaesthesia ward (28.4%) and were intervened within cardiology speciality (20%). Although easy to use, further development of Data collection form is needed if we are to continue with this research.
