Sažetak
Background: Psychopathology of the sensory experience is a significant indicator of the degree of psychiatric disorder, but at the same time it is an important parameter in differential diagnosis in the accurate assessment between psychical states. It can be a critical factor in the treatment, as well as in the outcome of a specific psychiatric disorder. Aim: The aim of this patient's analysis is to associate mental changes in a patient with recent strokes, visibile also on the brain ct, in whom afferwards, still during the hospital treatment, certain minimal neurological lesions were detected in the final diagnosis. Methods: Psychiatric interview, psychological status, neurological examination, ct analyses of the brain, control neurological examinations. Results: The patient was hospitalized because of psychotic symptoms like delusions and paranoid ideas. When admitted, he had a normal neurological status. CT of the brain performed five days after admittance showed several small recent ischemic lesions situated in the left region of the thalamus and in the right region of the putamen, along with signs of central atrophy. During a control neurological examination, 14 days after the admittace, mild right hemiparesis was discovered. Conclusion: Although the patient primarily seemed psychotic (with rich psychopathology of schizoophrenia-like disorder), CT analysis of the brain pointed towards a distinct orghanic background, what afterwards resulted in a neurological deficit and confirmed the different diagnostic dilemma.
