Sažetak
Aim: To explore the reasons for not prescribing anticoagulant therapy in secondary prevention of stroke. Patients and methods: A retrospective review of patients who were admitted to the Cerebrovascular Department of the General Hospital "Sveti Duh" in Zagreb, with atrial fibrillation and diagnosed stroke, in the period of three years. Results: 262 patients were included. Among these, 240 patients had ischemic stroke, 14 patients had intracerebral haemorrhage and 8 patients had haemorrhagic stroke. Before admission to hospital 58 patients had anticoagulant therapy (22.1%), 74 patients had antiplatelet therapy (28.2%). From a total of 166 patients who survived ischemic stroke, in secondary stroke prevention anticoagulant therapy was prescribed for 81 patients (48.8%) and antiplatelet therapy for 79 patients (47.6%). From 85 patients who did not receive anticoagulant therapy, 22 were patients with previous bleedings (mostly gastrointestinal), 13 patients had severe condition with poor prognosis, 18 patients had severe psycho-organic changes, 2 patients refused anticoagulant therapy, 3 patients were in very advenced age (>88 years). For 27 out of 85 patients (31.2%) no reasons were found for not prescribing anticoagulant therapy. Conclusions: The reasons for not prescribing anticoagulant therapy are not in all cases medically reasonable. Better awareness of advantages and adverse events probably could lead to more adequate usage of anticoagulant therapy. In our clinical practice, with exception of bleeding affinity, for not prescribing anticoagulant therapy the greatest impact has the patient’s age, general and psychic status.
