Sažetak
The video illustrates laparoscopic splenectomy after traumatic rupture of the spleen in a pregnant patient. The patient was a 25 years old female who was 18 weeks pregnant. She presented to the hospital after sustaining blunt abdominal trauma in a high speed motor vehicle accident.She arrived with signs and symptoms of hemorrhagic shock. Prompt resuscitation was started and immediate US examination performed which revealed a presence of free fluid in abdomen. Subsequent diagnostic laparoscopy revealed large quantity of blood in abdomen. Further exploration showed a lesion of the hilum of the spleen. Laparoscopic splenectomy was performed with the aid of a vascular stapler and a drain was placed. The patient had an uneventful recovery and was discharged on the eight postoperative day, with no discernible effects on the course of the pregnancy. We feel that for selected patients with traumatic splenic rupture diagnostic laparoscopy/splenectomy can be done safely and with good results after adequate resuscitation.
