Rationale: Optimal occlusion is not always the prime objective of patient in orthodontic treatment. However, when orthognatic surgery is proposed as a treatment option, there is still certain fear and rejection present in patient within our region. This case report presents the orthodontic and surgical correction of a Class III long face patient who was treated first for dentoalveolar compensation but with unsuccessfully aesthetics results. Diagnosis and etiology: A 21 year old female with the fixed orthodontic appliances and with an compensated skeletal Class III malocclusion. Her chief complaint was poor facial aesthetics. Treatment objectives: After levelling and decamouflage treatment, 3 mm of maxillary advancment by a high Le Fort I osteotomy, the mandible set back by bilateral sagittal split ramus osteotomy and a genioplasty for the vertical and horizontal reduction of the chin. Orthodontic treatment for 10 more months after surgery. A fixed retainer and Essix appliances for retention. Treatment results: After treatment favourable changes were noted in the occlusion, function and the soft tissue profile with the correction of the sceletal relationship. The patient was very satisfied with the treatment outcome. Conclusion: Combined orthodontic treatment with orthognatic surgery of a severe skeletal deformities allows achievement of an optimal aesthetic and functional results.
