Sažetak
A patient was admitted to our hospital with symptoms of vomitting, abdominal pain and respiratory dispnea. He had injuri in the car crash six month ago. We immediately preformed chest roentgenogram which confirmed prevously suspected strangulated diaphragmatic hernia. Hernia contained of stomach, colon, major omentum and spleen in left hemitorax which were compressing left lung. Diagnosis was confirmed by CT scan. After creating pneumoperitoneum, abdominal cavity was explored. We repositioned major omentum, colon and stomach through the diaphragmatic defect. Spleen was tightly fixed in toracal cavity and its reposition was not possible to achieve and we were forced to performe spleenectomy. The diaphragmatic defcet was repaired with interrupted sutures and toracal and abdominal cavity were drainaged. The postoperative course was uneventfull. Control CT scans of toracal cavity showed complete reexpansion of the left lung. We did not register any postoperative complications. Laparoscopic treatment of diaphragmatic hernia is save and reliable operative approach that can be performed in emergancy cases with complicated clinical condition because of all its well known advantages.
