Sažetak
Male patient, 66 years old, has suffered from chronic dacryocystitis with acute episodes and poor response to antibiotic therapy. He developed acute phlegmonous inflammation of right orbit and wassubjected to echographic examination. Peculiar echographic picture of orbital cellulitis with partial sound propagation into the ethmoidal space and total reflection on a highly echogenic surface inside lacrimal sac was found. MR also revealed spreading of the orbital inflammatory process into the ethmoidal cells. CT showed ethmoiditis and cellulitis presuming orbital lymphoma. Surgical intervention was performed by a team of ENT specialist and ophthalmologist with standard approach to the lacrimal sac. Distended sac filled with yellowish, hard plate (1, 5x1 cm) was found. Ethmoidal cells were partially excohleated and DCR performed. Postoperative course was uneventful and PHD analysis demonstrated Actynomyces Israelii inside the calcified contents giving the explanation for poor antibiotic response and peculiar echographic picture.
