Objective: Prehypertension is considered a condition that precedes hypertension, it carries an increased cardiovascular risk and is frequently associated with metabolic syndrome. There is a difference in the threshold blood pressure (BP) values for prehypertension between European and American experts (120-139/80- 89 mmHg for JNC 7, 130-139/85-89 mmHg for ESH/ESC). Our aim was to analyze the features of metabolic syndrome in prehypertensive groups based on two different criteria JNC 7 and ESH/ESC. Design and Method: Data obtained from 919 subjects (407 men and 512 women) who were enrolled in population based cohort study conducted in rural area were analyzed. After clinical examination BP and heart rate (HR) were measured using OMRON devices, fasting blood samples were drawn and spot urine samples were collected. Based on the BP values the subjects were divided as follows: group I (BP < 120/80 mmHg), group II (120-129/80-84 mmHg), group III (130-139/85-89 mmHg), group IV (140- 159/90-99 mmHg), group V (160-180/100-110 mmHg), group VI (BP > 180/110 mmHg) and group VII (systolic BP > 140, diastolic BP < 90 mmHg). We compared groups II and III, ANOVA and LSD post hoc analyses were used in statistical analysis. Results: There were no differences (p < 0.05) between two prehypertensive groups in body mass index (BMI), fasting blood glucose (FBG), total cholesterol (TC), HDL, LDL, triglycerides (T). The only difference was significantly higher values of waist circumference (p = 0.026) observed in the ESH/ESC prehypertensive group. However, subjects with optimal BP (group I) had lower values of BMI, FBG, WC, TC and LDL than subjects with normal and high normal BP (groups II and III, p < 0.05). Conclusions: We failed to find differences in metabolic parameters between prehypertensive groups based on two different criteria. However, we found that subjects with optimal BP have significantly less metabolic disturbances.
