Background: Colonic postinflammatory polyps in inflammatory bowel disease (IBD) patients reflect healing of ulcers and are often multiple. There are some data that ulcerative colitis has much often pseudopolys as a complication of the disease than Crohn colitis (CC). Pseudopolyps have no malignant potential but polypoid and pseudopolypoid manifestations of inflammatory bowel disease can be a source of great confusion. Differentiation of adenocarcinoma and dysplasia from inflammatory or postinflammatory polyps is sometimes very difficult. For this reason, endoscopy and patohistological analysis are necessary for making a final diagnosis. Aim: No reports on the prevalence of pseudopolyposis in Croatian inflammatory bowel disease (IBD) patients have been published. This study aimed to evaluate the pseudopolyposis complicating IBD patients managed at University Hospital Rebro (a tertiary referral center for inflammatory bowel diseases). Materials and methods: The clinical backgrounds of 238 IBD patients - 146 (61%) with Crohn's colitis (CC) and 92 (39%) with ulcerative colitis (UC) who have been treated in our division were analysed retrospectively. The diagnosis of pseudopolyposis was confirmed macroscopically, documented by reports and endoscopic pictures, and microscopically. Patohistological analysis was done by pathologist dedicated to IBD. Pathological reports excluded dysplasia and adenocarcinoma in IBD patients with pseudopolyposis. Results: We confirmed that 39/238 pts (16%) have colonic pseudopolyposis with no difference regarding to gender (male/female ratio 20/19, 51% ; 49% respectively). UC patients have pseudopolyposis in 19/92 pts (20%) ; Crohn colitis patients have pseudopolyposis in 20/146 pts (13%) (χ 2 test=1, 02 ; p value NS). In the group of UC pts there are 31 with left-sided disease and 61 with pancolitis. In the left sided group indidence of pseudopolyposis is 29% (9/31), and in pancolitis 11% (10/61) (χ 2 test 0, 75, p value NS). Conclusion: There is no difference in prevalence of pseudopolyposis in patients with ulcerative colitis and Crohn colitis in our cohort, also no difference in incidence regarding the initial extension of the disease in UC. We can conclude that phenotype (UC:CC) isn't relevant for the development of colonic pseudopolyposis.
