Sažetak
Tonsillectomy and tonsillotomy usually administered surgical procedures in the pediatric population. Also tonsillectomy is the first surgical procedure in the treatment of OSAS and (obstructive sleep apnea syndrome) in children. The technique of removing lymphatic tissue palatine tonsils developed during the long history of blunt dissection with the finger to the application of modern technologies such as electrosurgical blades, radio frequency and ultrasonic instruments with the aim of less tissue trauma and prevent intraoperative and postoperative complications. In this paper we compare the advantages and disadvantages of two surgical techniques removal of palatine tonsils as a cause of OSAS in the pediatric population. Over the past five years in our Department, in surgery of the head and neck, we use ultrasound instruments in various surgical procedures, and the past years in the treatment of OSAS and in adults and children. In the group of children with OSAS-infection and recurrent or chronic tonsillitis, tonsillectomy was performed - complete removal of lymphatic tissue palatine tonsils ultrasonic scissors or knife. In the group of children with symptomatic tonsillar hypertrophy underwent ultrasound intracapsular resection palatine tonsils - tonsillotomy. Ultrasound instrument proved to be a good surgical tool to minimally heat-traumatize the tissue tonsillar arches and loggias, the operative field is always without bleeding, shortens the time of surgery, and significantly shortens the postoperative recovery. The presence of postoperative pain was slightly higher in the group of tonsillectomized children with ultrasonic knife, but a group of children operated classic, but this difference was not statistically significant. A group of children who were decorated tonsillotomy ultrasound had significantly less postoperative pain.
