Sažetak
Pulmonary manifestations of systemic diseases are common. Arthritis rheumatoides (RA), systemic sclerosis (SSc), systemic lupus erythematosus (SLE), polymyositis/dermatomyositis (PM/DM) and Sjögren syndrome (SS), mixed connective tissue diseases, ankylosing spondylitis are diffuse connective tissue diseases with most often pulmonary manifestations. Group of vasculitides with pulmonary involvement are Wegener granulomatosis, Churg-Strauss and Goodpasture syndrome, furthermore Crohn disease and ulcerative colitis in group of inflammatory bowel diseases are also included. Cytologic findings can be specific like LE cells finding in pleural effusion of patients with systemic lupus erythematosus (SLE), , or mixture of spindle-shaped macrophages, round to elongated multinucleated giant cells and mixed inflammatory population of neutrophils, lymphocytes and small mononuclear macrophages for rheumatoid arthritis. Cytomorphology can be non-specific, as elements of granulomatous disease in exfoliative and FNAs samples as a marker of pathologic presentation, but not etiology of certain disease. BALF cytology and cell count can be helpful in differential diagnosis of interstitial lung diseases as well as additional special techniques as morphometric and DNA cytometric analysis of macrophages nuclei in BALF. The main task of presenting article is an overview of the cytological possibilities and diagnostic problems in various samples and materials obtain during diagnostic procedures in other to establish pulmonary manifestations of these systemic diseases.
