Sažetak
It is well known that there is strong connection between kidney function and bone diseases. Patients with CRF develop secondary hyperparathyreoidism. An excess of PTH shifts balance between osteoblast and osteoclast activity in favor to osteoclast activity and therefore bone resorption. To evaluate osteoblast and osteoclast activity we have measured iPTH, bone specific alkaline phosphatase (BAP), tartrate.resistant acid phosphatase isoform 5b (TRAP 5b), osteoprotegerin (OPG) and serum receptor activator of NF-kappaB - ligand (sRANKL). Seventeen renal graft recipients were enrolled at the moment of transplantation (Tx0) and followed up 6 (Tx6) and 12 (Tx12) months after. The control group consisted of 23 healthy individuals without known bone or renal disease. Control group iPTH median was normal. Highest iPTH values were in Tx0 group. Other two transplantation groups showed decreasing tendencies of iPTH values so we could say that iPTHvalues normalizes a year after transplantation. BAP median in control group were below assay median. posttransplantation groups showed increased BAP values compared both to control and Tx0 group (Tx6 vs control p<0.02 ; Tx12 vs control p<0.001 ; Tx12 vs Tx0 p<0.001). Median of TRAP 5b in controls was 2, 0 U/L. The highest value of TRAP 5b was noticed in Tx12 group. Tx12 TRAP 5b median vs control was statistically significant (p<0.001). In Tx0 group OPG median was highest (7, 3 pmol/L) and afterwards values successively decreased.
