Sažetak
Introduction. Recent studies suggest that CD4+CD25+FOXP3+ lymphocytes may bear immune tolerance toward kidney allograft. Aim. Aim was to determine concentration of CD4+CD25+FOXP3+ regulatory T lymphocytes (TREG) among peripheral mononuclear lymphocytes (PMNL) in stable cadaveric kidney transplant recipients. Patients and methods. Study included functionally stable 53 patients (23 males ; 50±12 years in age), with the first cadaveric kidney transplant received before at least a year (median 4 years, min. 1, max. 22). Patients suffered of miscellaneous basic kidney diseases and were treated with standard immunosuppression. Their GFR (expressed as creatinine clearance) was 68±27 ml/min. They underwent regular check-ups in a single centre. PMNL were isolated on Ficoll-paque density gradient and fenotyped by flow cytometry using monoclonal antibodies against CD4, CD25 and intracellular transcription factor FOXP3. Results. Peripheral leukocyte count was 7.8±2.4x109/l, 28±9% of them were lymphocytes. Among isolated PMNL there was 38.92±8.88% CD4+, 2.36±0.72% CD4+CD25+ and 2.06±0.65 CD4+CD25+FOXP3+. CD4+CD25+FOXP3+ presented 88±12% of CD4+CD25+ lymphocytes. There was significant positive correlation between concentrations of CD4+, CD4+CD25+ and CD4+CD25+FOXP3+, and linear regression between CD4+CD25+ and CD4+CD25+FOXP3+ (R-square=0.78). No gender difference was found. Age significantly positively correlated with concentrations of CD4+CD25+ and CD4+CD25+FOXP3+ cells (p=0.009), respectively. Time after transplantation, donor’s age, dialysis duration, HLA mismatches number or medication did not affect TREG concentration. Those received kidney from donors having blood group 0 had significantly higher concentrations of CD4+CD25+FOXP3+ lymphocytes, but also CD4+ and CD4+CD25+ cells. Conclusion. Stable kidney recipients had normally distributed concentration of CD4+CD25+FOXP3+ TREG, around 2% of PMNL. Most of the CD4+CD25+ lymphocytes were also FOXP3+. It seems that higher immune tolerance potential could favour older age of recipients.
