Sažetak
Occlusal caries of permanent molars is problem in every day practice because that tooth surface is still most frequently affected by caries. Fissures may act as reservoirs for cariogenic bacteria and preventive measures should be taken to control microbial activity at these sites. Purpose of this study was to evaluate effect of two different fissure sealants on prevention of occlusal caries in permanent molars. This study was conducted on two examined and one control group. In first group, 392 teeth in 98 patients, aged 7-9 years, were sealed with resin based material (Helioseal F, Ivoclar Vivadent, Liechtenstein). In second group, 260 teeth in 65 patients, same age, were sealed with glass ionomer cement (Fuji Triage, GC Corp., Japan). In control group, 300 teeth were examined in 75 patients of the same age, whose permanent molars were never sealed. Control examinations of sealed teeth were conducted after 6, 12 and 24 months. Retention of the sealants was controlled as well as occurrence of caries on occlusal surfaces in all patients. Results were statistically analyzed using t- test and χ2 test. At the end of the study, resin based sealant material showed much better retention in where complete retention measured 72% in comparison to glass ionomer sealant which retention was 19%. Meanwhile, occurrence of new occlusal caries lesions at the end of study was 25% in resin based and 30% in glass ionomer group without statistically significant difference between groups. In control group, 65% of occlusal fissures had active occlusal caries or filling. We can conclude that pit and fissure sealing is valuable method in prevention of occlusal caries and that both tested materials could be used with good preventive effect.
