Sažetak
Augmentation and immediate implantation have not been previously recommended in infected sites, however recent studies showed significant improvements in the reliability and predictability of such procedures. We report of a 67-year-old female patient, who required multiple tooth extractions and fabrication of new overdentures in the upper and lower jaw. Ridge-split osteotomy, augmentation and immediate implantation in the infected areas followed extraction. Augmentation was performed with easy-graft CLASSIC (Degradable Solutions AG, Schlieren, Switzerland). Four Blue Sky implants (Bredent GmbH & Co.KG, Senden, Germany) were placed in the maxila in the position 15, 13, 23 and 25. No membrane was placed over the augmented sites. In the mandible two Blue Sky implants implants were placed in the positions 33 and 43. The sites were sutured and a temporary denture was fabricated. Amoxicilin and metronidazol antibiotics were prescribed for a period of 10 days. After a period of six months x-ray analysis showed no signs of inflammation or problems in implant osseointegration. The implant sites were reopened and healing abutments were placed. Impressions were made using Impregum Penta Soft (3M ESPE, St.Paul, Minn, USA) polyether material. In the maxilla an individual bar retained overdenture and in the mandible a ball retained overdenture were manufactured. After six months clinical examination and x-ray analysis showed good implant health and bone resorption was within standard values. Immediate implantation and augmentation in infected sites is still a much debated topic but can be a reliable and predictable procedure if surgical protocols are followed. Although research is showing good results in such cases, further research such as randomised clinical trials are required for a definite conclusion.
