The degree of internal carotid artery (ICA) stenosis is important predictive factor for ipsilateral ischaemic stroke. Two multicentric trials showed benefit of carotid endarterectomy in patients with severe carotid stenosis. Both trials were based on angiography. Noninvasive imaging have to set criteria for degree of carotid stenosis. The aim of this study was to compare the degree of carotid stenosis diagnosed by color Doppler flow imaging (CDFI) with digital subtraction angiography (DSA). We analyzed the data of 91 patients admitted to neurology department who suffered TIA, amaurosis fugax or stroke. CDFI of carotid arteries was performed and the degree of stenosis was classified as mild, moderate, severe and occlusion. The ultrasonographic criteria for mild stenosis were: diameter reduction <50%, sistolic blood flow velocities (SBFV) <1.7 m/s, for moderate stenosis: diameter reduction 51-75%, SBFV 1.71-2.99, severe stenosis (two of the following): diameter reduction 76-99%, SBFV >3 m/s, negative flow in ophtalmic artery. For DSA NASCET criteria were used. The data of 91 patients (61 males, 30 females) were analyzed. In the group of mild stenosis CDFI classified 52 and DSA 55 carotids; in the group of moderate stenosis CDFI 43, DSA 29 carotids; in the group of severe stenosis CDFI 53, DSA 59 carotids; and as occluded CDFI classified 34 and DSA 39 carotids. Sensitivity of CDFI for detecting severe carotid stenosis was 75%, and the specificity 98%, accurate diagnosis 88%. The sensitivity of CDFI for detecting carotid occlusion was 82%, and the specificity 0.98%, accurate diagnosis 95%. The Spearman's Rank Correlation Coefficient was 0.86. CDFI is a sensitive and specific method for diagnosis all degree of carotid stenosis
