Sažetak
Necrobiosis lipoidica is a granulomatous condition with a degenerative connective tissue that is often resistant to therapeutic intervention. Several reports have demonstrated successful management with corticosteroids, immunosuppressants, anti-inflammatory treatment or surgical excision. This case report demonstrates a fast progressive necrobiosis lipoidica in a patient with cardiomyopathic disease. Due to the extensive lesions on face and systemic cardiac disease that excluded systemic therapy a novel topical approach was required. Therefore, intralesional triamcinolone acetonide and topical pimecrolimus was tried for two months. The combination of corticosteroid and immunomodulatory treatment resulted in complete and sustained remission. This combination of topical drugs has not been previously reported in the treatment of necrobiosis lipoidica.
