Sažetak
In this study of a large cohort of patients with type 1 diabetes we provided the evidence that reduced insulin sensitivity is associated with increased urinary albumin excretion. Insulin sensitivity was more strongly related to albuminuria than with serum creatinine and creatinine clearance. Reduced insulin sensitivity is more pronounced in diabetic patients with microalbuminuria, and it precedes the onset of microalbuminuria. Microalbuminuria is one of several risk factors for end-stage renal disease in diabetic patients. It is also an indicator of organ dysfunction and a marker of greatly increased cardiovascular morbidity and mortality in patients with either type 1 or type 2 diabetes mellitus. Since progression to microalbuminuria is likely to occur in majority of diabetic patients, there is a need to further explore the role of risk factors such as insulin resistance.
