Sažetak
In this study of a large cohort of patients with type 1 diabetes we provided the evidence that cigarette smoking is associated with the development and progression of early diabetic renal damage (albuminuria). Cigarette smoking was more strongly related to albuminuria than with serum creatinine and creatinine clearance in our patients. It is possible that smoking could influence renal tubule and permeability functions independent of any specific effects on clinical propensity to nephropathy. Microalbuminuria is the earliest clinical indicator of diabetic nephropathy, and 25-80% of patients with type 1 diabetes and microalbuminuria go on progress to higher rates of urinary albumin excretion associated with progressive kidney disease. Cessation of smoking slow progression to renal failure, but the decision to stop smoking is difficult because of the high addictive potential of the habit. The challenge remains for diabetologists and nephrologists to motivate patients to stop smoking.
