Sažetak
Objective: The aim of the study was to investigate the role of Helicobacter pylori (HP) infection and NSAID using on bleeding complications in duodenal ulcer (DU) and gastric ulcer (GU). Method: 906 patients suffering from DU (579) or GU (327) were submited to gastroscopy. Within patients with DU 136 had bleeding complication. HP status was confirmed with histology, urea breath test (UBT) and CLO-test. Anamnesis about NSAID using during at least 7 days prior bleeding incident was taken before admiting to gastroscopy. Incidency of bleeding complications in HP positive and negative NSAID users and non users were compared with chi-square test for DU and GU. Results: Among bleeding DU patients, 80 were using NSAID and 125 were HP positive and 70 were HP positive and taking NSAID. Within patients with GU 83 had bleeding complications. 45 bleedig GU were using NSAID, 72 were HP positive and 40 were HP positive and were using NSAID-s. pos neg pos neg NSAIDpos HPpos 40 87 p=0.0349 70 81 p=0.3600 HP neg 5 32 S 10 17 NS NSAID neg HPpos 32 83 p=0.0310 54 298 p=0.0000 HP neg 6 42 S 4 44 NS HPpos NSAIDpos 40 87 p=0.3900 70 81 p=0.0000 NSAID neg 32 83 NS 55 298 S HP neg NSAIDpos 6 42 p=0.8900 4 44 p=0.0022 NSAID neg 5 32 NS 10 17 S Discussion: In patients suffering from bleeding DU, there is signifficantly higher incidency of NSAID users among HP positive and HP negative patients. In NSAID users and nonusers there is significantly higher incidence of bleeding from GU, in HP positive patients than in HP negative.
