Sažetak
Cardigenic shock is the most serious complication of acute coronary syndromes and cause of death in 4.2-7.2% of patients with acute ST-elevation myocardial infarction (STEMI). Cardiogenic shock mortality rate of 80% in patients treated conservatively was not significantly decreased with introduction of thrombolysis, especially streptokinase in the treatment of STEMI. Introduction of primary PCI reached that goal. Since 2000 in Departement for cardiovascular diseases, Clinical hospital “Sestre milosrdnice” permanently was in charge urgent interventional cardiological team. In that time 839 patients were hospitalized with diagnosis of STEMI, 391 of them were treated with primary PCI, 70 (8, 3%) of them developed cardiogenic shock, in 33 STEMI was treated conservatively and in 37 with primary PCI. According to results of this study cardiogenic shock had frequency of 8.3% in patients with STEMI. Those patients are on average older, with higher percent of female, higher percent of inferior and expecially right ventricular myocardial infarction, and culprit leasion more often in right coronary artery in comparision with other patients with STEMI. However, in patients who died as result of cardiogenic shock, anterior wall of left ventricle and left anterior descendent coronary artery are more often affected. Treatment with primary PCI decreased mortality rate of cardiogenic shock on 36 %. Characteristics of STEMI in patients with cardiogenic shock are different in comparison with average STEMI. Primary PCI is, without doubt, therapy of choice in treatment of this serious complication of acute STEMI .
