INTRODUCTION: According to literature, reperfusion arrhytmias that cause hemodynamic instability or subjective discomfort in acute myocardial infarction (AMI) should be treated METHODS AND PATIENTS: Authors investigated the frequency of different types of therapy used in reperfusion arrhytmias (frequent premature ventricular complexes, non-sustained ventricular tachycardia, accelerated idioventricular rhythm, high-degree atrioventricular blocks, ventricular fibrillation, transient sinus bradycardia and sustained ventricular tachycardia) in 147 patients (pts.) hospitalized in Coronary Care Unit (CCU) with diagnosis of STEMI and treated with urgent PCI or thrombolysis. RESULTS AND DISCUSSION: One third of investigated pts had to be actively treated because of reperfusion arrhytmias. Lidocain, electrocardioversion and atropin were the most often used treatments, while temporary electrostimulation or amiodaron were rarely used. Most of the pts who had need for active therapy of reperfusion arrhytmias were from urgent PCI group (42.7% vs. 21.5%), especially those with lidocain in therapy. Authors concluded that reperfusion arrhytmias in pts with STEMI, which appear after urgent PCI, were more serious than those after thrombolysis.
