BACKGROUND. The incidence rate of early-stage prostate cancer has dramatically increased since the introduction of prostate-specific antigen (PSA) screening in developed countries. Nowadays, besides PSA levels and Gleason score, age is considered to be a key prognostic factor in terms of treatment decisions. The present study wanted to establish the differences of clinico morphological parameters for prostate cancer between group of elderly and young men treated with radical prostatectomy according to low and intermediate risk EAU criteria. PATIENTS AND METHODS. A database of 97 men aged ≥65 years and 134 aged ≤65 years was reviewed to compare the clinico pathological features between the groups which were surgically treated in the period from 1998 and 2009. Pretreatment PSA, Gleason score, and postoperative Gleason score, pT stage, and positive surgical margins were correlated. RESULTS. The median age for a group of elderly patients was 69 years while the group of younger patients was 60 years. Younger group had a statistically significant higher preoperative Gleason score (p = 0.015 ; ), with the median value of 6 (range 2-9). Other parameters were not statistically different between two groups. DISCUSSION AND CONCLUSION. The preoperative prognostic factors identified by histological analysis of needle biopsy prostate specimens Gleason grading score, serum markers (PSA), and clinical staging could predict pathological stage and outcome of individual patients with clinically localized prostate cancer. In our study significantly higher preoperative Gleason score in younger patients was presented differently from other studies, which revealed that there were no diversity regarding preoperative variables, however, younger patients have lower prostatectomy Gleason scores and less extra-prostatic extension and positive surgical margins. In our study overall positive surgical margin rate was 34, 32 % what is in concordance with the results of large studies indicating that the positive rate of surgical margin after radical prostatectomy is 20%–40% (4). We can conclude that finding of significantly higher preoperative Gleason score in the group of men younger than 65 years suggests that these patients were still included in the population of surgically treated patients.
