Sažetak
PURPOSE: To report on a case of bilateral corneal ulcer followed by acute corneal melting in a child that was hospitalized in intensive care unit and had a Pseudomonas aeruginosa infection, together with treatment measures made to save child˘s vision.
METHODS: Patient was on systemic antibacterial treatment prior to admission to Ophthalmology Department. Upon corneal ulcer formation, conjunctival swab showed P. aeruginosa infection. Bilateral corneal melting required Ťa chaudť total keratoplasty bilateraly, combined with lensectomy and vitrectomy on the right eye. Surgical results, postoperative treatment, follow-up and visual outcome in first three postoperative months are presented.
RESULTS: Corneal graft remained clear and without any sign of graft rejection on the right eye. Three weeks following first surgery, due to the graft oedema and intensive inflammatory reaction in the anterior chamber causing pupilar closure, rekeratoplasty was performed on the left eye.
At the end of our follow-up, final visual acuity of counting fingers on one meter distance was achieved bilateraly.
CONCLUSIONS: According to our knowledge this is the first case of bilateral corneal melting in eyes with P. aeruginosa infection occuring in only six hours.
