Sažetak
Preoperative diagnostic curretage in patients with stage I endometrial carcinoma may cause positive finding in peritoneal washing (PW) due to retrograde dissemination of cancer cells into the peritoneal cavity. Therefore, it may affect recurrence of disease and cancer survival rate. The aim of the study was to estimate if preoperative curretage is associated with higher incidence of positive PW in patients with stage I endometrial carcinoma, and to compare the postoperative recurrence of disease in relation to PW finding. Cytological analysis of intraoperative PW was performed in 41 patients (aged 65+/-8.7 yrs) undergoing surgery for stage I endometrial carcinoma. In 31 patients, diagnostic curretage was performed prior to surgery. PW findings were classified as positive (PW+), negative (PW-), or suspect (PWs) for malignant cells. In PWs, immunoreaction on epithelial antigen (EA) was performed. Chi-square test was used to determine whether curretage is related to higher incidence of PW+, and to determine the significance of PW+ in disease recurrence. Of all patients, 34 patients were PW-, 7 were PW+ (2 were classified as positive after positive immunoreaction on EA). The mean interval between curretage and surgery was 19 days. There was no significant difference in PW+ ocurrence between two patient groups that did (3/31) and did not (4/10) underwent curretage preoperatively. The mean interval of postoperative follow-up was 3.5 years. In this period, 5/41 (12.1%) patients developed recurrent adenocarcinoma. The PW+ patients had a significantly higher incidence of disease recurrence (3/7), in relation to PW- (2/34) (p<0.05). One PW+ patient died during the follow-up. In our study, the 3.5-year recurrence-free survival rate was 91.3% and disease specific survival rate was 97.6 %. Preoperative curretage is not associated with a higher incidence of positive peritoneal washing, while positive PW is significantly related to disease recurrence in patients with stage I endometrial carcinoma.
