Sažetak
Objectives: Objective of this report was to present a fracture of a full arch implant supported metal-ceramic restoration. Metal-ceramic restorations are still the most commonly used implant suprastructures. Functional durability of these restorations depends on a number of factors. Overloading, insufficient framework design, casting failures, different coefficients of thermal expansion between the alloy and the veneering ceramics can all lead to failure. Methods: We report of a 63 year old female patient with a completely edentulous mandible and maxilla. The patient was happy with her complete upper full denture, but wished for a fixed restoration in the mandible. Six implants were placed, four blueSKY implants width 4.0 length 14mm (Bredent GmbH & Co.KG) in the posterior and two XiVE implants width 3.0mm length 13mm (DENTSPLY Friadent) in the anterior of the mandible. Impressions were made using an individual tray with a polyether material. A full arch metal-ceramic restoration (Cr-Co alloy, Wirobond® 280 Bego, Germany and IPS Classic, Ivoclar-Vivadent, Liechtenstein) was fabricated and provisionally cemented using eugenol-free luting cement. Results: After 14 days an inspection was made and a complete fracture of the ceramics and metal framework between pontics was found. We believe that the failure occurred due to the fracture of the metal construction and we analysed the possible causes. An insufficient metal connective area between pontics of 1.9 mm bucco-lingualy was found. Conclusion: Metal framework has a major role in the functional durability, stability and strength of metal-ceramic restorations. Strength also depends on the bond between the metal construction and ceramics, shape and rigidity of the cast and the compatibility between alloys and ceramic. Connective area between pontics is the most common place of metal framework fractures. Insufficient area size is the main reason for there fractures.
