Sažetak
With the aging of population and the progressive lengthening of life expectancy, the number of elderly patients who suffer trauma will continue to grow. Elderly patients (older than 65, WHO 1963.) who suffer trauma and emergency procedures have a higher mortality. However, chronological age is much less important as an independent risk factor ; a more important risk predictor of perioperative mortality is the presence of coexisting disease. All people suffer a decline in organ function as they age, affecting autonomic responsiveness and the endocrine response to stress, as well as the more obvious major organ systems. Many maintain homeostasis perfectly well, some despite chronic disease. However, when they are traumatized they have a decreased ability to maintain homeostasis or to regain it if lost. Every effort in anesthesia should be made to perform a thorough preoperative evaluation, including nutritional assessment, and to optimize the status of the patient's chronic medical diseases as much as possible before surgery. Recently, several studies have identified albumin level to be a good predictor of postoperative mortality. Albumin, a marker of nutritional status, may serve as a surrogate marker for the preoperative health status of the surgical geriatric patient. The aims of anesthesia in intraoperative period are hemodynamic stability, avoiding hypothermia and blocking endocrine response to stress. Anesthesia care in postoperative period includes multimodal concept: patient control analgesia, early mobilization, early enteral nutrition and wide indication for intermediate and intensive care unit admission.
