Sažetak
Industrial components must comply with the requirements for reliability and safety throughout their entire life cycle. Anybody taking part in their design, production and/or operation may be responsible for a possible failure. The types of responsibility are design responsibility, production responsibility and exploitation (operation) responsibility. This paper presents two actual cases of failure: the case of production responsibility where the bearing girders of Diesel engines in several large tankers cracked, as well as the case of design responsibility where a failure of a redesigned wheel on a high speed train failed caused a disaster. The authors want to point out that, in order to fulfill the reliability and safety requirements for a component, proper care is to be taken about the following influences of utmost importance: the actual operating loads and usage conditions, the design shape and proper dimensioning, the material, as well as the production process.
