Objectives: Atopic dermatitis is a chronic inflammatory skin disease characterized by eczematous skin lesions that affect 10-20 % of the population. This disease results from interaction between multiple tiegger factors known to exacerbate the condition. Diagnosis of AD is based on careful history, recognition of typical clinical manifestations and additional tests. In most cases skin tests, total IgE and allergen sopecific IgE determination are sufficient for detection of sensitization to various inhalant and food allergens. However, a complex diagnosis in some patients is difficult due to age, skin alterations, apllication of drugs, or the results of widely used tests are equivocal. In this study, we eevaluated the diagnostic reliability of cellular antigen stimulation test (CAST-ELISA) in detection of allrgic reaction to inhalant and food allergens in patients with atopic dermatitis. Methods: We tested 91 patients at the age from 3 months to 65 years with a history of hypersensitivity reactions to inhalant abnd food allergens. Sulfidoleukotrienes (sLT) quantification was carried out by CAST-ELISA test after in vitro peripheral blood leukocytes stimulation with inhalant and food allergens. Results: Our results show that CAST-ELISA test was positive in 84% patients with allergy to Der p, 70% of patients to Der f, 55% of patients to grass pollen, 50% of patients to ragweed pollen, 56% of patients to birch pollen, 29% of patients to milk, 79% of patients to eggs, 46% of patients to soya and 42% of patients to wheat. Conclusion: Our results suggest that CAST test proved to be safe and useful in the routine investigation of hypersensitivity reaction to inhalanat and food allergens.
