Sažetak
Transcranial Doppler (TCD) sonography was used to assess blood flow velocity (BFV) in patients with acute stroke aged 35-78 years. All patients were admitted within 24 hours from the onset of stroke. Neurologic examination revealed hemiplegia and cranial nerve paresis. Computed tomography (CT) eliminated intracranial hemorrhage, while extracranial occlusive disease was excluded by extracranial Doppler examination. In all patients, CT findings were consistent with ischemic stroke in the middle cerebral artery (MCA) irrigation area. BFV was investigated in both MCAs, both anterior and posterior cerebral arteries, and in the carotid siphon. Initial assessment was performed within 24 hours from the onset of disease, and compared with the neurologic finding. Second examination was carried out within 7 days, and third 3 weeks after the onset of stroke, compared with the neurological outcome. The values obtained were compared with reference values. The examinations showed typical TCD hemodynamic disturbances in the early period of stroke. MCA occlusion was found in 20% of patients, and in 50% of them gradual vessel patency was monitored by TCD. The patients in whom BFV in MCA within 24 hours was >30 cm/s had better prospects of survival and suffered less disability. The patients with MCA occlusion and BFV <30 cm/s in other vessels had poor prognosis. A BFV of 30 cm/s was confirmed to be the lower limit required for blood perfusion.
