Introduction: Prevalence of HPV infection is high compared to the number of women who develop cervical cancer. Methods based on E6/E7 mRNA detection may be superior to DNA methods, to identify patients with potential to develop cervical cancer. Methods: This study included 81 cervical sample collected into Thin-prep medium, 48 high-grade squamous intraepithelial lesion (HSIL), 13 low-grade squamous intraepithelial lesion (LSIL), 8 atypical squamous cells of undetermined significance (ASCUS), 7 atypical squamous cells (ASC-H) and 5 with normal cytology. E6/E7 mRNA expression was detected by the NucliSENS EasyQ assay and presence of HPV DNA by the Linear Array HPV Genotyping Test. Results: With HPV DNA assay, all cervical samples gave positive results whereas 48/81 (59.3%) samples were found positive with mRNA assay. HPV DNA was detected in 45/48 (93.6%) patients with HSIL, in 10/13 (76.9%) patients with LSIL, in 5/8 (62.5%) patients with ASCUS, in 7/7 (100%) patients with ASC-H and in 2/5 (40%) patients cases with normal cytology. The E6/E7 mRNA was detected in 37/48 (77.08%) patients with HSIL, in 5/13 (38.46%) patients with LSIL, in 3/8 (37.5%) patients with ASCUS, in 3/7 (42.85 %) patients with ASC-H, and 0/5 patients with normal cytology. Discussion: A small proportion of HPV DNA positive women with LSIL, ASCUS and ASC-H had a detectable E6/E7 mRNA expression, while mRNA was not detected in women with normal cytology. E6/E7 mRNA based tests may be more appropriate for risk evaluation.
