Sažetak
Rickettsioses are obligatory notifiable diseases in Croatia. According to data from the Croatian Institute of Public Health 174 cases of Brill-Zinsser disease, recrudescent form of louse-borne epidemic typhus caused by Rickettsia prowazekii, were recorded in Croatia from 1957 to 2008. Most of the patients gave a history of epydemic typhus during the period from 1941 to 1953, and more than half of them originated from Bosnia and Herzegovina. Sporadic cases of endemic (murine) typhus, which occurred in or originated exclusively on the Croatian littoral and islands between Zadar and Split, have been observed nearly every year. Seroprevalence to R. typhi among healthy population living on coastal part and on the islands around Zadar were between 23.0% and 66.0%, and in the area of Split 21.4% . Human cases of Mediterranean spotted fever (MSF) have been observed in the southern part of Croatia from Zadar to Dubrovnik, most frequently during the summer. Serologic evidence of circulation of R.conorii among inhabitants was obtained in all the surveyed localities in southern Croatia. R. conorii was isolated from the blood of one of our patients, as well as from the tick vectors Rhipicephalus sanguineus, Rh. bursa and Dermacentor marginatus. In 1991 R. akari, agent of rickettsialpox, was isolated from blood of a patient diagnosed with MSF in Zadar. Results from subsequent seroepidemiological investigation showed that 24.8% persons living in the area of Zadar and adjacent islands had antibodies to R. akari. By using molecular techniques in different tick species the presence of tick-borne R.slovaca, R. aeschlimannii, and R. helvetica were detected. Sporadic cases, minor or major epidemics of Q fever have been described mostly from January till May in Istria, Primorje, north Adriatic islands, Dalmatia and north Croatia, where Q fever is an endemic disease. The purpose of the present review is to summarize the data of the current knowledge about rickettsiae and rickettsioses in Croatia.
