Sažetak
The development of community based rehabilitation is the main objective of the project implemented in partnership between Croatian and Canadian professionals in leading institutions in Zagreb (Sveti Duh Hospital) and Kingston (Queen's University). The project has been supported by the World Health Organisation, the Canadian Government through CIDA, the Government of the Republic of Croatia through the Ministry of Health and the city of Zagreb through the Office for Health, Labour and Social Protection.
Clinical practice based on the principles of community based rehabilitation was introduced as a demonstration model in Croatia. The model was developed through the creative collaboration of Croatian and Canadian experts and introduced the concept of a multidisciplinary team and the principles of continuing education and health promotion. The theoretical concept of community based rehabilitation was applied in practice because of the common interest of persons with disabilities and rehabilitation professionals to use a new approach to rehabilitation. A key factor to the successful development of the program was the communication and information sharing between stakeholders. At the health policy level, the model successfully demonstrated a new approach and the possibility for more effective and efficient functioning of the existing health and social system.
