Sažetak
Measurements of renine activity in plasma is used in diagnosis of certain forms of hypertensia. Renine activity in plasma is elevated in patients with renovascular hypertensia. It is well documented that the cause of renovascular hypertensia is obstruction (aneurysm) of renal artery or its larger branches, which leads to lower renal prefusion, higher tubular resorption and the activation of renine-angiotensine-aldosteron system. The best method for this diagnosis is renal angiography and measurement of separate renines. During renal angiography, a probe is introduced through femoral veins until renal veins or its bigger branches from where blood is taken for separate renins measurements. Because this method is very invasive it is not widely used, but instead it is used only in patients with higher renine activity during performance of captopril test. Diagnostic value of the test is significant increase in plasma renine after the administra¬ tion of captopril (15-50 mg) dependent on the patient's weight. Measurements of renine activity using captopril test was done on 37 year old patient and higher values were obtained. The patient was subjected to renal angiography and the samples were collected for the measurements of separate renines. Higher renine values obtained implied stenosis of the left renal artery. The patient was sent to a surgery. However, once more higher renine activity values were obtained using both captopril test and repeated renal catheterization, implying the obstruction of the right renal artery. In conclusion, the measurement of the renine activity in plasma is reliable screening method for detection of renovascular hypertensia caused by obstruction (aneurism) of renal artery or its bigger branches.
