Sažetak
Cochlear implants, aimed to make deaf person to become able to use acoustic means of communication, ask for a long-range patient rehabilitation. This rehabilitation needs an interdisciplinary team of experts, trained to use sophisticated technical tools, but also able to understand cognitive aspects of language acquisition. Till now, cochlear implants were placed to 3 adult patients at the Šalata Otorhinolaryngology Clinic in Zagreb. Their speech and hearing rehabilitation is performed by making them able to detect, discriminate, recognize and understand acoustic signals. The goal of the rehabilitation is to learn patients to read lips better, to control own speech well, and to recognize and understand speech signals coming from the environment. This paper is a case study on these 3 patients (2 prelingual, 1 postlingual), where we elaborate our original method developed for purpose of training for speech understanding. Training and testing samples were composed of vowels, numbers and common words frequently used in Croatian (Vuletić). The most important test parameter was intelligibility. The intelligibility is defined as a ratio of number of patterns correctly repeated by patient and total number of emitted patterns. Using sound and lip reading performed basic training. Words used for training were never used for testing. By testing, each patient was brought in normal environment, without acoustic isolation, exposed to noise and echo, and with speech source placed behind his/her back. Contrary to conventional test samples applied on patients with implant, we decided to use words as patterns. Words (common nouns, verbs, and adjectives) are basic elements of meaning, and we regard the communication as an exchange of meaning between communicating subjects.
