Aim of study: The aim was to determine the prevalence of elevated aminotransferase levels and potential prevalence in Croatian type 2 diabetic population. Those data serve as an estimate of the percentage of apparently 'simple' type 2 diabetic patients seen in common outpatient practice requiring further diagnostic work-up and referral to a hepatologist Methods:Case records from Sestre Milosrdnice University Hospital's Endocrinology department's diabetes outpatient clinic were reviewed using data from Crodiab database. All pts with type 2 diabetes and available transaminase levels data were included in the study. Prevalence of elevated aminotransferase levels was calculated and correlation between elevated ALT levels and various parameters (sex, age, BMI, hyperlipidemia, HbA1c levels, diabetes therapy, alcohol consumption) was analyzed. Results: 1011 case records were reviewed, and 913 patients with type 2 diabetes were included in the study. The prevalence of elevated ALT levels was 16.3% (145/888 pts), where 13.9% (123/888 pts) had ALT<2x elevated and 2.5% (22/888) had ALT>2x elevated. Only 3.5 % (5/145) of those patients admitted to more than occasional alcohol consumption. There was no statistically significant association between presence of elevated ALT levels and analyzed parameters. Patients with elevated ALT levels had a tendency towards less frequent ALT elevation in patients on metformin therapy (11.5 % vs. 17.1 %, n=686, P=0.07), which is in accordance with previously reported effects of metformin on improvement in aminotransferase levels. Conclusion: Elevated aminotransferase levels are a frequent finding in Croatian type 2 diabetic patients. A question that remains is how many of those patients are actually referred to a hepatologist and have a thorough work-up done to confirm NAFLD and exclude other causes of chronic or acute liver damage.
