Sažetak
Aim of study: To establish wheter body mass index (BMI), waist and hip circumference correlate with the severity of acute pancreatitis and can be taken in account as risk factors. Methods: A hundred and nine patients with first attack of acute pancreatitis were analyzed, mean age 63, 2&#61617; 14, 7, 53 women, age 65, 98&#61617; 13, 79 and 56 men, age 60, 57&#61617; 15, 14. BMI, waist and hip circumference were measured for all patients. APACHE II (mean = 8, 1&#61617; 3, 3), Ranson (mean = 2, 64&#61617; 1, 58) and CTSI (mean = 2, 07&#61617; 2, 48) scoring systems were assessed in all subjects, as well as the lenght of hospital stay (median = 15 days ; range = 1 &#8211; 35). Local and systemic complications were defined according to Atlanta criteria. Local complications were found in 28% and systemic complications in 14% of patients. We performed correlations between BMI, waist and hip circumference and the above mentioned scoring systems and complications using the Spearman correlation test. Results: We found statistically significant correlations of waist and hip circumference with APACHE II (p<0.000002 ; p<0.00009) and Ranson (p<0.0006 ; p<0.03) scores. The CTSI score correlated significantly only with waist circumference (p<0.04). A significant prolongation of hospital stay was found in patients with higher waist and hip circumference (p<0.02 ; p<0.004). Our results suggest a significant influence of BMI, waist and hip circumference on systemic complications (p<0.02 ; p<0.001 ; p<0.01), but no influence on local complications. BMI correlated significantly only with APACHE II score (p<0.001), while no correlation was found with other parameters. Conclusion: BMI, waist and hip circumference have a statistically significant influence on the occurrence of systemic complications in patients with acute pancreatitis, while we found no influence of the same parameters on local complications.
