Introduction: The aim of our work was to evaluate APACHE-O score, by comparison to APACHE-II, in predicting severe acute pancreatitis (SAP). Furthermore, we analyzed the role of obesity as a single predictive factor for severe outcome. Methods: We analyzed a group of 173 patients admitted for acute pancreatitis (AP) to our clinic from January 2004 to December 2006. APACHE-II score on admission was recorded for each patient. APACHE-O score was calculated by adding 1 point (BMI between 26 and 30 kg/m2) or 2 points (BMI>30 kg/m2) to the APACHE-II score. The severity of acute pancreatitis was assigned according to Atlanta criteria. Results: Out of 173 patients, 50 (29%) were obese (BMI > 30). Twenty six patients developed SAP (11 in obese and 15 in non obese group). Accuracy of APACHE-O and APACHE-II in predicting SAP were 56% and 60%, respectively, both at cut off value of 8. Furthermore, logistic regression analysis showed that BMI > 30 is a significant risk factor for organ failure in AP (OR = 2.5, p = 0.047). Conclusion: In our study we have shown that APACHE-O is not superior to APACHE-II in predicting the severity of AP. Therefore, adding a BMI score to APACHE-II score is not useful. We have also shown that obesity, measured as BMI score is a single predictor of severe AP.
