Sažetak
Objective: To evaluate the rate of anxiety disorders in Parkinson’ s disease (PD). In addition, we investigated the association between anxiety symptoms and clinical features of PD. Background: Anxiety is a common experience in patients with PD occurring in 40% of patients. Despite frequent occurrence of anxiety and significant contribution to morbidity, anxiety symptoms in PD received relatively little attention Patients and Methods: A total of 150 idiopathic PD patients ( 82 men, 68 women) were enrolled in the study. Depressive and anxious symptoms were rated using the Hamilton Anxiety Scale (HAM-A) and Beck Depression Inventory (BDI). Patients that scored more than 18 points in the HAMA-A were consideres to have anxiety disorders. Clinical evaluations consisted of Unified Parkinson’ s Disease Rating Scale (UPDRS part I, II, III, IV), Hoehn and Yahr (H&Y) staging and Parkinson’ s disease quality of life 39-item questionnaire PDQ-39 (PDQ-39). Results: The proportion of PD patients that were considered to have some form of anxiety disorders was 47%. The comorbidity of mood and anxiety disorders was found in 24 % PD patients. Anxiety occurred more often in female PD patients and patients with young onset of PD as compared to other. Anxiety symptoms correlated with several variables: a) disability variables including UPDRS part II (Spearman rho 0.27, p<0.05) and PDQ-39 (Spearman rho 0.62, p< 0.01) b) motor variables including UPDRS part III (Spearman rho 0.25, p< 0.05) and H&Y (Spearman rho 0.34, p<0.05) c) neuropshyciatrics variables including UPDRS part I (Spearman rho 0.54, p<0.01) and BDI (Spearman rho 0.59, p<0.01) d) variables regarding motor fluctuations including duration of “ off” periods (Spearman rho 0.38, p< 0.01) and occurrence of freezing (Spearman rho 0.22, p< 0.05). The use of antidepressant medication was higher in patients that had anxious disorders as compared to others. Discussion: Anxiety occurs often with depression and has significant effect on patient’ s quality of life and clinical features of the disease. Clinician should pay special attention to this group of patients.
