There is epidemiological evidence that raised blood pressure is associated with an impaired prognosis in patients with coronary heart disease (CHD). Therefore, blood pressure control is crucial in these high risk patients. Repeated studies in Europe revealed that blood pressure in coronary patients is insufficiently controlled. In the present study we aimed to evaluate current blood pressure management and control and its time trends in coronary patients across Europe. In EuroAspire III, a study undertaken in 2006-2007, patients with CHD<80 years of age from centres in 22 European countries were included. Standardised interviews and examinations were conducted on average 1.2 years after patients’ coronary events. Blood pressure was measured twice in a sitting position on the right upper arm in 8946 patients and the mean of the two measurements was used in data analyses. In non-diabetic (diabetic) patients the blood pressure target was defined as systolic blood pressure <140 (<130) mmHg and diastolic blood pressure <90 (<80) mmHg. Mean systolic (diastolic) blood pressure in patients from EuroAspire III was 140.3 (82.7) mmHg. Overall, 56.0% of participants showed a raised blood pressure ranging from 30.8% to 68.4% between centres. Proportion of men (women) with blood pressure on target amounted to 45.6% (39.3%) and decreased with increasing age (60.6% at age<50 years to 35.9% at age 70 years). Almost all patients (94.8%) used at least one blood pressure lowering agent irrespective of the purpose of prescription whereas 70.2% of patients reported use of medication specifically prescribed to lower blood pressure. Among all patients using blood pressure lowering agents 43.9% (range 31.3% to 69.6% between centres) had their blood pressure controlled. Among patients using drugs specifically prescribed to lower blood pressure 37.3% (range 21.3% to 65.2%) achieved blood pressure control. Prevalence of undetected hypertension defined as the proportion of patients unaware of their raised blood pressure was 20.0%. Time trend analysis in the 8 centres participating in all three EuroAspire surveys showed a persistently high prevalence of raised blood pressure (1994--1995: 58.1%, 1999--2000: 58.3%, 2006--2007: 60.9%). Prevalence of raised blood pressure in patients with CHD persisted at a high level between EuroAspire surveys. Despite the widespread use of antihypertensive drugs the proportion of patients achieving blood pressure control is insufficient.
