Sažetak
Pharmacists care of patients with diabetes and persons at high risk for the development of chronic diseases can help significantly in the education of general public and reduction of chronic disease complications and consequently treatment costs. Fundamental forms of pharmacists care of diabetic patients are the counseling on healthy life-stile, adequate drug usage and follow-up of treatment course aimed at improvement of disease and quality of life. The Vuk Vrhovac University Clinic and the School of Pharmacy from Zagreb have therefore organized continous training of pharmacists on diabetes mellitus.
A total of 237 pharmacists from the whole of Croatia have been educated so far. In addition to lectures, practical work in workshops has been organized, enabling pharmacists to practice work with diabetes related tools. Knowledge test performed initially and at the end of the course indicates a marked improvement (p 0.001) and also increased interest in participation in chronic patients education. A pharmacists thus becomes a part of health team delivering care to patients with diabetes (early disease detection, prevention of the development of acute and chronic complications). Based on such a strategy a CRO PharmaDiab Program was developed as part of the National program concerning pharmacists participation in diabetes care and of the EuroPharm Forum Project.
